SCHOLARSHIP APPLICATION FORM
To apply for any of the scholarships - Mozart Scholarship, Gulf Coast Harpers Memorial Scholarships or Outstanding Performance Scholarship, complete the following Scholarship Application Form midnight on MAY 1st.

TEACHER RECOMMENDATION FORM required for Mozart Scholarship and Gulf Coast Memorial Scholarships.
Applicants are required to have their teacher submit the Teacher Recommendation Form by midnight on MAY 1st to complete the scholarship application.
Outstanding Performer Scholarship application does NOT require this form.
